Normal Renal Ultrasound Report Template
Normal renal and bladder ultrasound report template with kidney measurements, echogenicity, hydronephrosis and post-void residual wording.
EXAMINATION: Renal ultrasound. CLINICAL HISTORY: [indication] COMPARISON: None available. TECHNIQUE: Real-time grayscale and color Doppler imaging of the kidneys and urinary bladder. FINDINGS: Right kidney: [x] cm. Normal echogenicity and cortical thickness. No hydronephrosis, calculus or mass. Left kidney: [x] cm. Normal echogenicity and cortical thickness. No hydronephrosis, calculus or mass. Bladder: Normal wall thickness. No intraluminal lesion. Post-void residual: [x] mL. IMPRESSION: 1. Normal renal ultrasound. No hydronephrosis.
Dictation notes
- Always give both renal lengths — serial sizes are how chronic disease is followed.
- Report the post-void residual as a number when the study was ordered for retention.
Frequently asked questions
What belongs in a normal renal ultrasound report?
A renal ultrasound report gives each kidney's length, echogenicity and cortical thickness, and explicitly states the absence of hydronephrosis, calculus and mass for each side. The bladder is assessed for wall thickness and intraluminal lesions, and a post-void residual is reported when measured. The normal impression is 'Normal renal ultrasound. No hydronephrosis.'
Why are both kidney lengths reported on renal ultrasound?
Renal lengths should be stated as numbers for both kidneys because serial size is how chronic kidney disease is followed on imaging — progressive loss of length and cortical thickness tracks parenchymal loss. A single 'normal size' statement cannot be compared against later studies. Asymmetry between sides is itself informative, raising questions such as renal artery stenosis or chronic scarring.
When should a post-void residual be reported as a number?
Whenever the study was ordered for urinary retention or voiding symptoms, the post-void residual should be reported as a measured volume in milliliters rather than a qualitative impression. The number is what referring clinicians act on — it guides catheterization decisions and follow-up — and it provides the baseline against which treatment response is judged. Bladder wall thickness and any intraluminal lesion are reported alongside it.
