Normal CT Soft Tissue Neck Report Template (With Contrast)
Normal contrast-enhanced CT neck report template: pharynx and larynx by subsite, salivary glands, thyroid, nodal stations by level, vascular and visualized chest, with notes on abscess, nodal and incidental thyroid wording.
EXAMINATION: CT soft tissue neck with intravenous contrast. CLINICAL HISTORY: [indication, e.g. “Neck mass”] COMPARISON: None available. TECHNIQUE: Axial helical images from the skull base through the thoracic inlet after the intravenous administration of [x] mL of [contrast agent], with coronal and sagittal reformats. Dose reduction techniques were used. FINDINGS: Nasopharynx: Symmetric. No mass. The fossae of Rosenmüller are patent. Oral cavity and oropharynx: The tongue base, palatine tonsils and soft palate are symmetric without mass or asymmetric enhancement. The parapharyngeal fat planes are preserved. Hypopharynx and larynx: The epiglottis, aryepiglottic folds, false and true vocal cords are symmetric. The pyriform sinuses and postcricoid region are unremarkable. The paraglottic and pre-epiglottic fat is preserved. The laryngeal cartilages are intact. The airway is patent. Salivary glands: The parotid and submandibular glands are symmetric and unremarkable. No ductal calculus or ductal dilatation. Thyroid: Normal size and homogeneous enhancement. No nodule. Lymph nodes: No pathologically enlarged or necrotic cervical lymph node by size or morphology at levels I-VII bilaterally. No retropharyngeal node. Retropharyngeal and prevertebral spaces: Normal thickness. No fluid collection. Vessels: The carotid arteries and jugular veins are patent and normally opacified. No thrombus. Spine: No acute osseous abnormality. No destructive lesion. Visualized lung apices and mediastinum: Clear. No apical nodule. Visualized brain and skull base: Unremarkable. IMPRESSION: 1. No acute or mass-like abnormality of the soft tissues of the neck. No cervical lymphadenopathy.
Dictation notes
- A rim-enhancing low-attenuation collection is a “possible abscess versus phlegmon” unless it has a complete, well-defined wall — overcalling drainable fluid sends a patient to the OR for a dry tap, and the report should give a size in three dimensions.
- Localize every abnormal node by level (I-VII) and describe necrosis, size (short axis) and matting — a necrotic level II node in an adult is a metastasis from an oropharyngeal primary until proven otherwise.
- Any incidental thyroid nodule on a neck CT is reported under the ACR white paper — ultrasound for a nodule over 1.5 cm (under 35 years: over 1 cm) or with suspicious features; nodules below threshold need no follow-up and the report should say so.
- For a laryngeal or pharyngeal mass, map the subsite spread (paraglottic fat, pre-epiglottic fat, cartilage invasion) rather than describe a “mass” — the T stage is built from those lines.
